By electing the Vitality GO rider, the Insured acknowledges that they will receive a membership in a healthy engagement program offered by a third party program provider and authorizes the Company to share his/her personal information, including certain health information, with the provider in connection with the registration for the program and administration of the Vitality GO rider. The Insured understands and agrees that (i) his/her program membership will be subject to the provider’s privacy policy and terms and conditions of membership, which the Insured should read prior to joining the program, and (ii) he/she will be asked to authorize the provider to share his/her health, lifestyle, medical or other personal information with The Company. The Insured will not be eligible to participate in the program if the terms and conditions of membership are not accepted. Upon termination of the Vitality GO rider, the program membership will terminate and access to further benefits and incentives, if any, will cease as provided in the terms and conditions. The Company is not responsible or liable for any damage, loss or injury arising out of the Insured’s participation in any third-party healthy engagement programs or receipt of any products or services provided through such programs.
By electing the Vitality GO rider, you agree to accept electronic delivery of the Healthy Engagement Core (Vitality GO) Rider to your life insurance policy. To provide the consent, you will need a device with access to the internet in addition to Adobe software as described below. You should print (or store locally on your own device) and maintain a copy of the Healthy Engagement Core Rider document presented to you as part of this electronic life insurance transaction process. We also recommend that you have the capability to print copies of certain online screens and documents presented through the process.
You also have the option of completing the life insurance transaction through our offline process utilizing paper forms. If you do not wish to complete this life insurance transaction electronically, simply click on the "x" in the upper right-hand corner of the screen to close the internet browser window on your computer and call John Hancock at 888-333-2659. In addition, if you provide your consent and then withdraw it, we cannot process your transaction electronically.
