Authorization to Share Information and Consent to Electronic Record Delivery and Communications

Authorization to Share Information

By electing the Vitality GO rider, the Insured acknowledges that they will receive a membership in a healthy engagement program offered by a third party program provider and authorizes the Company to share his/her personal information, including certain health information, with the provider in connection with the registration for the program and administration of the Vitality GO rider. The Insured understands and agrees that (i) his/her program membership will be subject to the provider’s privacy policy and terms and conditions of membership, which the Insured should read prior to joining the program, and (ii) he/she will be asked to authorize the provider to share his/her health, lifestyle, medical or other personal information with The Company. The Insured will not be eligible to participate in the program if the terms and conditions of membership are not accepted. Upon termination of the Vitality GO rider, the program membership will terminate and access to further benefits and incentives, if any, will cease as provided in the terms and conditions. The Company is not responsible or liable for any damage, loss or injury arising out of the Insured’s participation in any third-party healthy engagement programs or receipt of any products or services provided through such programs.

Consent to Electronic Delivery of Vitality GO Rider

By electing the Vitality GO rider, you agree to accept electronic delivery of the Healthy Engagement Core (Vitality GO) Rider to your life insurance policy. To provide the consent, you will need a device with access to the internet in addition to Adobe software as described below. You should print (or store locally on your own device) and maintain a copy of the Healthy Engagement Core Rider document presented to you as part of this electronic life insurance transaction process. We also recommend that you have the capability to print copies of certain online screens and documents presented through the process.

You also have the option of completing the life insurance transaction through our offline process utilizing paper forms. If you do not wish to complete this life insurance transaction electronically, simply click on the "x" in the upper right-hand corner of the screen to close the internet browser window on your computer and call John Hancock at 888-333-2659. In addition, if you provide your consent and then withdraw it, we cannot process your transaction electronically.

Consent to Electronic Communications

Until you withdraw your consent, as described below, you agree to the following:

  • To receive the Healthy Engagement Core Rider during the electronic life insurance transaction process. By proceeding with the electronic life insurance transaction process, you will receive the Healthy Engagement Core Rider electronically in accordance with the terms of this consent.
  • To receive emails from John Hancock Life Insurance Company (U.S.A.) and its affiliates (“John Hancock”) regarding products and services at the email provided as well as transactional disclosures, communications, and notices related to your Account, including those that are otherwise required by applicable law to be provided to you in writing necessary for the servicing of my current policy, such as premium statements (collectively, “Communications”).
  • You confirm that you have the required hardware and software described below under the heading “Hardware and Software Requirements” and the ability to access, download, save and/or print Communications made available to you.
  • You confirm that you have a valid e-mail account that you are able to access. You have the sole responsibility of providing John Hancock with a correct and operational email address. John Hancock will not be liable for any undelivered email Communications. You must promptly notify us of any change in your email address.
  • You acknowledge that it is your responsibility to download and save or print Communications for your records.
  • You agree that John Hancock may discontinue sending paper documents and provide Communications to you electronically. However, we may continue to send certain insurance documents via paper as required by law, or as determined by us in our sole discretion.

Hardware and Software Requirements. To access, download, save and/or print Communications electronically you must have: you must have:

  • A computer suitable for connecting to the Internet with a Current Version (see description below) of an Internet browser we support.
  • To access Communications through a mobile website or mobile application, one or more devices, such as a smartphone, mobile device or tablet, suitable for connecting to the Internet and for downloading mobile applications and accessing mobile websites with a Current Version of one of the mobile operating systems we support (see description below);
    • a connection to the Internet;
    • a valid e-mail address that you are able to access and e-mail software or web-based access;
    • the ability to access documents in HyperText Markup Language (HTML) and Portable Document Format (PDF) (in order to access PDF documents, you must have a Current Version of software that accurately reads and displays PDF files, such as Adobe Acrobat Reader). If you don’t have Adobe Acrobat Reader on your computer, it can be obtained, free of charge, by clicking on the following link: Adobe Reader Download; and
  • Local, electronic storage capacity to retain our Communications and/or a printer to print Communications; and

By “Current Version,” we mean a version of the software that is currently being supported by its publisher.

Withdrawal of Consent to Receive Electronic Notices.

Your consent to electronic delivery of Communications remains in effect until you give us notice that you are withdrawing it. You may withdraw your consent to electronic delivery of Communications at any time by calling us at 800-387-2747. Your withdrawal of consent will become effective as soon as we have a reasonable opportunity to act upon it. At our option, we may treat your provision of an invalid e-mail address or phone number, or the subsequent malfunction of a previously valid e-mail address or phone number, as a withdrawal of your consent to receive Communications electronically.

How To Obtain Paper Copies.

You understand that you can obtain a paper copy of your Communications by printing it yourself or by obtaining a paper copy from John Hancock at no charge by calling 800-387-2747 and requesting a copy.

By electing the Vitality GO rider, I confirm to John Hancock that:

  1. I have read, understand and agree to the terms and conditions in this Consent; and
  2. I can send and receive emails with hyperlinks to websites and attached files.
