Personal Information Request Form
If you are a John Hancock customer and have been provided online access to your account/policy/plan, you can log in to view your personal profile. As a resident of California, you have certain rights to request access and/ or to delete your personal information that we may have on you or your household.
In order to view your request, we need to be able to 1.) authenticate you and 2.) locate any personal data for you or for whom the request is being made. There may be instances when we may be unable to fully comply with a request. When this occurs, the company will provide a written response with the reasons.
