Establish provider eligibility
In addition to verifying benefit eligibility, John Hancock will verify that each long-term care service provider meets the policy criteria for a qualified provider. The care provider eligibility process can begin as soon as you start a claim.
Adding a new provider
If you need to update information related to providers you listed during the claim initiation process, or add new providers, please sign into your account and send the following information via "Messages and Documents”:
- Care provider name
- Care provider address
- Care provider phone number
- Type of care
- Provider email address
- Start of care date
Please check with your provider(s) for any special release forms that may be required in order for John Hancock to obtain the necessary information regarding your claim.
If using an Independent Care Provider
If using an Independent Care Provider (ICP), you can use the CareGiver mobile app. This digital solution makes it quicker to track care and submit reimbursement requests — without the need for paper forms and physical signatures.
