John Hancock Vitality: Prevention and Early Detection

1. Beginning Veteran's Day 2025 and running through at least July 4, 2026, for every four new Vitality registrations, John Hancock will sponsor one Galleri test which will be donated directly to Home Base.

The Galleri test is manufactured and distributed by GRAIL. John Hancock is not involved in the design or manufacture of the Galleri test and is not responsible for the accuracy or performance of the Galleri test. Galleri is not a test to confirm or rule out genetic or other conditions that may indicate a predisposition to cancer. The Galleri test is currently not approved by the U.S. Food and Drug Administration.

The Prenuvo scan is developed and administered by Prenuvo and its affiliated professional entities. John Hancock is not involved in the design or administration of the Prenuvo scan and is not responsible for the accuracy or performance of the Prenuvo scan.

The lab testing services available via Function Health are administered by third party lab services providers . John Hancock is not involved in the services provided by Function Health and is not responsible for the accuracy or performance of such services.

Galleri is a blood test from Grail designed to screen for cancer. The Galleri test does not detect all cancers nor does it diagnose cancer. A test result of “Cancer Signal Detected” requires confirmatory diagnostic evaluation by medically established procedures (e.g., imaging) to confirm cancer. There may be additional costs for such diagnostic testing. Rx only. It is intended to be used in addition to, and not replace, other cancer screening tests recommended by a healthcare provider. A negative test result does not rule out cancer and in particular the sensitivity rate for detecting cancer in the early stages is lower than for later stages. A false positive or false negative result can occur and results should be discussed with a healthcare provider. Test eligibility is subject to certain qualification requirements, including age and certain health risk factors.

The Prenuvo scan should not be considered as a replacement to any standard clinical guidelines-based screening for specific types of cancer or other health conditions. The Prenuvo scan is subject to the completion of an MRI safety assessment as well as a medical intake review by Prenuvo. Please contact your own healthcare provider with any specific questions regarding your health or medical conditions. As with any medical test, there are limitations, which make it impossible for the Prenuvo scan to detect all malignancies and disease conditions. It is also possible that there may be findings that upon further diagnostic testing are not significant or might not require immediate medical intervention. Function Health does not offer medical advice, laboratory services, a diagnosis, medical treatment, or any form of medical opinion, through their services or otherwise. Function Health’s services are not a substitute for medical care, medical advice, and/or a detailed discussion with your primary care physician or other licensed provider. All laboratory and medical services are provided by independent third parties of Function Health. There may be additional costs for diagnostic testing. There is no coordination between John Hancock and any health benefits you may receive from an insurance policy, health plan, or any other wellness programs you may be enrolled in.

Your results will have no impact on your current life insurance coverage. That said, like other medical tests you elect, a decision to take the Galleri test, the Prenuvo scan or one or more lab tests made available through Function Health, and the results, may need to be disclosed in an application for insurance coverage, unless prohibited by law, and thus could affect your access to future insurance coverage and the price of such coverage.

John Hancock does not receive individual results from Grail, Prenuvo or Function Health, nor does it know which customers have taken the test, scan or lab tests. For purposes of research, validation of assumptions and evaluation of future health and wellness offerings, John Hancock may receive aggregated and deidentified data from Grail, Prenuvo and Function Health from time to time.

Access to the Galleri test, Prenuvo scan or the Function Health membership through the John Hancock Vitality Program is not currently available in all states and is subject to change.

The Galleri test is only available to registered John Hancock Vitality PLUS members who are 40 years of age or older and have completed the initial Vitality Health Review (VHR). For eligible polices with Face Amounts of $500,000 or greater in life insurance coverage, 100% of the cost of the test will be subsidized. For policies with Face Amounts less than $500,000 in life insurance coverage, 50% of the cost of the test will be subsidized. Test eligibility is also subject to Grail’s clinically informed criteria, as well as additional cancer risk factors for those ages 40-49. Please note: for members with multiple policies, the policy with the highest Face Amount in life insurance coverage will determine the subsidy amount of the Galleri offer.

The Prenuvo scan discount is only available to registered John Hancock Vitality PLUS and GO members who have completed the Vitality Health Review (VHR) for the current program year.

Access to special pricing for the Function Health membership is only available to registered John Hancock Vitality PLUS members who have completed the Vitality Health Review (VHR) for the current program year.
